PLAYER REGISTRATION for the
2011-2012 season:
- Register with USA Hockey. This registration is necessary for your insurance coverage. You will receive a confirmation email. Print this receipt. It needs to be submitted with your registration forms.
- Fill out the Code of Conduct. This is necessary for skaters PeeWee and up.
- Get a Sports Physical. This is required for all high school students. Turn in the physical with your registration forms.
- Consent to Treat. All players need to provide this document.
- Financial Aid Packet. Note that this is due September 15 for all travel league players and October 15 for all house league players. Payment plans must be submitted through the financial aid committee. Payment Plan Form
- Registration Forms. Select the correct form for your skater.
- (House League) (Travel League) Registrations are due October 1st for travel league players. House league players must register prior to skating.
- Play Up Requests. Play Up requests should be mailed as soon as possible but are due no later than October 1st.
- Mail Forms. Forms and fees can be mailed to Rushmore Hockey; PO Box 832; Rapid City, SD 57709 or deliver at one of our registration nights at the rink.
Registration Forms
Coaches: You need to include your CEP Data along with their USAHockey registration. Also - registerwith USA Hockey with the same name you have on yourcoaches card so that your coachinglevel will show on the state roster.
